Of the Hojo Fuonji branch, he served as Northern Rokuhara Tandai, maintaining order in Kyoto and overseeing the court. In 1333, Rokuhara fell to forces supporting Emperor Go-Daigo. He attempted to flee eastward with Emperor Kogon but was blocked at Banba-juku in Omi by Ashikaga and Sasaki forces. He committed suicide at Renge-ji with 432 retainers and kinsmen, at the age of 27.

A tragic warrior who bore heavy responsibility at a young age and remained loyal to his clan to the end.

Renge-ji in Omi preserves a death register of 432 names including Nakatoki, conveying the tragedy of the Kamakura shogunate's fall.
